What We Do

Our Programs

Three pillars of care — each built to meet real needs with compassion, dignity, and hope. From the hospital room to the dinner table, we're here.

Heart Failure Support

You Don't Have to Face This Alone

A heart failure diagnosis can feel overwhelming — for patients and families alike. Our Heart Failure Support program connects individuals with the resources, education, and peer community they need to navigate life with heart failure from diagnosis through every stage of care.

  • One-on-one patient navigation and resource referrals
  • Educational workshops on heart failure management
  • Peer support groups for patients and families
  • Assistance connecting to medical and financial resources
Caregiver Support

Caring for the Caregivers

Caregivers give everything — their time, energy, and love — to support someone they care about. Our Caregiver Support program provides relief, recognition, and resources so caregivers can sustain their own wellbeing while continuing to show up for their loved ones.

  • Caregiver respite resources and referrals
  • Support groups and community connection
  • Mental health and wellness resources
  • Practical assistance and care coordination guidance
Heart to Table Mobile Pantry

Nourishing Lives, One Meal at a Time

Welcome to the Heart to Table Mobile Pantry — an initiative of HCFO dedicated to combating food insecurity in our community. Every month, we host food distribution at Moody Civic Center in Moody, AL, where community members receive fresh produce, staple foods, and pantry items with dignity and respect. This program is in partnership with the Community Food Bank of Alabama.

Community Services

Stronger Together

Beyond our core programs, HCFO connects individuals and families to a broader network of community services — from housing and transportation assistance to social support and advocacy. We believe whole-person care means addressing every barrier to health and wellbeing.

  • Community resource navigation and referrals
  • Transportation assistance for medical appointments
  • Advocacy and awareness events
